Access Health and Community: Your Journey, Your Choice

Access Health and Community

At Access Health and Community, we care about your health needs. With over 150 years of experience in providing quality healthcare, we’re here to support you.

Your health journey isn’t just about medical appointments. Whether it’s a check-up, emotional support or connecting you to other services, we can help. We also assist carers and loved ones.

Allied Health Services: Support for Everybody

Our team doesn’t believe in a “one-size-fits-all” approach. We offer many flexible support options. Whether you’re visiting our clinics or would prefer home support, we can find the right plan for you.

Allied Health Services available:

  • Diabetes Nurse Education
  • Dietetics
  • Physiotherapy
  • Podiatry
  • Occupational Therapy
  • Speech Therapy

Our fully qualified staff offer excellent care. We are also experienced NDIS providers. We can provide NDIS reports, helping you secure ongoing support or apply for new services. NDIS clients can use our services at the clinic, at home, or in the community.

For people with Home Care Packages, we offer thorough and affordable treatment plans. This helps you save funding for other services.

Medication Support and Recovery Service (MSRS): Helping you Manage your Medication

If you’ve been prescribed more than one medication, sometimes it’s easy to lose track.

The MSRS is a specialist service for anyone who uses prescription and/or over-the-counter medications. We help people who might be worried about their medication use or thinking about making changes. We also support family members and carers.

We offer counselling, peer support and nursing assistance. Peer support workers can help you connect with others in your community and to activities you enjoy.

Our specialist nurse practitioners are experienced in working with complex health conditions such as chronic pain.

Nurse Practitioners Offer:

  • Health assessments
  • Medication management plans
  • Education and advice about medications
  • Prescriptions and pharmacotherapy options
  • Collaboration with your GP/healthcare providers

Our services are free. We don’t take funds from your NDIS or Medicare treatment plans.

Carer Support: a Break for You and Your Carer

Our Carer Support Service offers fun and affordable programs in your home or at our centres. You might want to meet people or try a new hobby. Our respite program helps you do these things whilst your carer has a short break.

Home-based Activities Available:

  • Transport to appointments
  • Going on outings
  • Talking or doing activities in the home
  • Completing household chores together

Center-based Activities:

  • Exercise groups
  • Painting, drawing and craft
  • Social support
  • Group outings

We also provide support to people who are feeling isolated or want help engaging with others.

We assist people living in the cities of Yarra, Darebin, Boroondara and Stonnington, who are registered with the NDIS or My Aged Care. Some supports are available on weekends or in the evening.
